Registration for Upskate NY Junior Roller Derby. To register, select the number of players you are registering and add to cart. Once you complete the checkout you will be redirected to the forms page.

Please fill out all forms for all players you are registering. If you select the Emergency Contact and Medical Form, you will be automatically redirected to the next form one after another.
To fill out for multiple children return to the https://upskatenyjrd.org/registration-requirements/ page and go through the forms again for each player.
